COVID-19 SCREENING TOOL
The following questions provides basic information only and they are NOT intended to take the place of medical advice, diagnosis or treatment. If you don’t pass the screening you should not book an appointment. If you have passed the screening questions below then you should use hand sanitizer and mask during your eye examination.
Results of Screening Questions:
• If you answer NO to all questions from 1 through 4 you have passed and can book an eye examination appointment . You need to wear a mask and use hand sanitizer before the eye examination. Please click here
• If you answer YES to any question from 1 through 3, you have not passed and cannot have eye examination at this point. Please stay at home to self-isolate immediately and call 250 380 2020 to schedule a virtual visit.
• If you answer YES to question 4 and you do not have a fever, you have passed and can book an eye examination. You need to wear a mask and use hand sanitizer before the exam and should self-monitor for COVID-19 symptoms. Please click here
